The CMS Transforming Episode Accountability Model (TEAM) is a bundled payment program for five types of surgical care that 740 hospitals will be required to participate in starting on January 1, 2026. Derek Haas, Founder & CEO, Avant-garde Health
Derek founded Avant-garde Health based on research from Harvard Business School. Since 2016, he has helped health systems and ASCs succeed with bundled payments. His work has been featured on the front page of The Wall Street Journal, and he’s co-authored 12 Harvard Business Review articles, including How One Hospital Has Succeeded in a World of Bundled Payments and Getting Bundled Payments Right in Health Care. He holds a BA and MBA from Harvard.

Ann Conrath, Director of Sales, Avant-garde Health
Ann has led the design and rollout of national value-based care programs, including maternity care home models and bundled payments, and brings deep experience in helping hospitals operationalize change. Before joining Avant-garde, she built go-to-market strategies for healthcare technologies across the U.S. She holds a BA from Marquette and an MSW from Washington University in St. Louis.

Dr. Porter Jones, Chief Medical Officer, Avant-garde Health
Dr. Porter began his VBHC work at Hospital Albert Einstein in Brazil, where he implemented case costing, tracked outcomes, and developed Centers of Excellence in high-acuity surgical specialties. At Avant-garde, he leads research and client strategy, helping hospitals adopt best practices in value-based care using analytics, predictive modeling, and AI. He holds an MD from the University of Utah and an MBA from Harvard Business School.

Cynthia Hundorfean, Senior Partner CEO Advisory Network
Cynthia Hundorfean is an innovative and trusted executive who has worked in bold strokes to get health care right for patients, members, communities and clinicians while navigating dynamic changes in the healthcare market.  She has a record of delivering explosive growth through transformational partnerships, strategic acquisitions and organic growth strategies.  Cynthia joined the CEO Advisory Network in August 2024, as a Senior Partner. Prior to that, she served for 7 years as the President and CEO of Allegheny Health Network (AHN), a health system generating over $5B in annual revenue with 23,000 employees, 2,500 physicians, 14 hospitals, and over 300 clinical locations.  After taking the helm in early 2016, she led the system to a return to profitability in 16 months, moving AHN from a $32.6M loss in 2016 to a net profit of $51M in 2017.  This was achieved through an aggressive move to value-based care, strategic cost reductions, consolidation of sites and creation of centers of excellence. ​
